Embracing Life's Transition
What is Hospice Care
Hospice care at Destiny Hospice focuses on symptom management and quality of life for individuals facing terminal illnesses. A team of nurses, social workers, spiritual chaplain, hospice aides, volunteers, and others work together to manage pain, provide companionship, offer comfort, and ease family caregivers’ burden.
Your care is physician-directed and tailored to each patient’s specific needs. Hospice care services are carried out wherever the patient lives, whether that be the patient’s home, a family member’s home, or a senior living facility where the patient resides, allowing those with a life-limiting illness to remain in a familiar setting surrounded by love and support. Care comes in the form of assistance with day-to-day living activities, symptom relief, and stress relief aimed at delivering peace during the course of care.
What is Palliative Care
For individuals living with a severe illness who are still undergoing care for that illness, palliative care services help improve the quality of life during treatment. The Destiny Hospice palliative care team focuses on symptom relief and support at any point after your diagnosis, and care is coordinated with your doctor to help you and your family cope with your illness.
